Case Management Enhances Home Care
CAI's case management team is trained to help clients caregivers, advocates and guardians cope with the demands of long term care situations. Our case management services are delivered by Nurses, Social Workers, and Service Coordinators.

CAI specializes in designing unique services to custom fit the needs of its clients.  If you don not see the specific service you are looking for, please call to discuss your needs.  We will make every effort to assist you and maximize your resources.

Case Management is for:

  • Complex home care situations
  • Families who live out of town and are not readily accessible
  • Individuals without family support
  • Individuals experiencing difficulty adjusting to enforced change
  • Families experiencing difficulty adjusting to the demands of care giving
  • Individuals unable to adhere to physicians orders or manage their medications
  • Professionals who are responsible for some aspects of a persons legal, financial, medical or environmental needs

Case Management Staff:

  • Assess the case and develops a plan of care
  • Oversees all aspects of home and client care as they relate to the plan of care
  • Regularly monitors service delivery and client well being
  • Arranges for additional services as needed
  • Coordinates care with physicians orders
  • Advocates for the client’s best interest
  • Acts as a liaison in the absence of family
  • Is available to oversee household management and operation
  • Helps families adjust to enforced change
